I did revive them with the scroll though. step 3 (very important to your problem) click somewhere not in death fog. So you only get those characters and if they all die I can't get them back? Because I already got some of my characters killed already in the begining of the game. you teleport the player as part of the revive process. You can very easily get the missed achievement on future playthroughs.
